Position Overview We are currently seeking a qualified Licensed Master Social Worker (LMSW) for the role of Nursing Home Surveyor/Complaint Investigator in New York, NY . This essential role focuses on ensuring quality of care and regulatory compliance in long-term care facilities through surveys, complaint investigations, and surveillance activities. Key Responsibilities Conduct surveys and complaint investigations in long-term care facilities, including cases involving abuse or neglect Evaluate nursing homes for compliance with Federal and State regulations Draft Statements of Deficiencies (SOD) in a timely manner using the Principles of Documentation Assess the effectiveness of Plans of Correction and ensure timely follow-up Enter findings into state and federal compliance databases Participate in Immediate Jeopardy monitoring and state interventions when needed Attend relevant meetings and testify in administrative hearings as necessary Follow all Department of Health (DOH) schedules, policies, and procedural guidelines Travel to assigned facilities according to work schedule Greenlife Healthcare Staffing Empowering Healthcare Professionals.
