About the Role:
As a Nursing Home Surveyor/Complaint Investigator, you'll play a critical role in upholding healthcare standards. You'll investigate complaints—including serious allegations like abuse or neglect—conduct surveillance activities, and assess regulatory compliance at nursing homes throughout the state.

Key Responsibilities:
Conduct surveys and investigations to evaluate facility compliance with Federal and State regulations
Draft clear, timely Statements of Deficiencies (SODs) using Principles of Documentation
Review and assess facilities' Plans of Correction
Document findings and enter data into state and federal reporting systems
Respond to Immediate Jeopardy situations and participate in monitoring
Attend relevant hearings and testify when necessary
Follow Department of Health (DOH) schedules, policies, and procedures
Travel to onsite facilities according to your assigned schedule

What You Bring to the Table:
Licensure: Active NYS Master Social Worker license (LMSW required)

Experience:
At least 2 years of clinical experience in a medical facility (nursing home experience strongly preferred)
Experience conducting surveillance activities is a plus
Certification: SMQT certification preferred (must be obtained within 12 months if not currently held)

Skills:
Strong writing and documentation skills
Solid judgment and ability to remain objective
Excellent interpersonal communication
Reliable, organized, and able to travel for on-site assessments
